School Based Health Clinic at FLS

We're excited to offer a School-Based Health Center at Frederick Leighton Elementary School. This partnership with ConnextCare will offer our students a wide range of medical, dental, and behavioral health services. 

What is a School-Based Health Center (SBHC)? 

The SBHC is a full-service health center operated by ConnextCare and partially funded by the State of New York. The SBHC is staffed with a Licensed Practical Nurse (LPN) and/or Medical Assistant (MA), and a Family Nurse Practitioner (NP) or Physician Assistant (PA). A supervising Pediatrician is regularly onsite and available should they be needed at any time. Behavioral Health Services are provided by Licensed Master Social Workers (LMSW).

Primary care services available to enrolled students include:

  • Treatment of illnesses
  • Treatment of minor injuries
  • Administration of immunizations
  • Comprehensive physical examinations in accordance with the American
  • Academy of Pediatrics
  • Monitor and treat chronic illnesses such as asthma and diabetes

Behavioral health services available include:

  • Individual counseling
  • Group counseling
  • Family counseling
  • Psychoeducational and behavioral case reviews
  • Substance abuse counseling

Dental Services available include:

  • Cleanings
  • Fluoride treatments
  • Sealants
  • Oral health education

What happens if my child is sick and the school is closed?

throughout Oswego County:

When is the SBHC open?

The SBHC is open when school is open. When school is closed the SBHC is generally closed, however, certain locations have summer and/or holiday hours.

How does my child receive services?

Call and schedule an appointment.
*If your child is home sick and school is open, your child can still be seen at the SBHC.

Send a note requesting an appointment with a number where you can be reached.

How do I enroll my child/children?

An enrollment form will need to be completed to register your children with the health center. Enrollment forms can be found on the ConnextCare website at www.connextcare.org, at school offices, or the SBHC located within Leighton Elementary School, . Should assistance be needed in completing or locating forms, we will be happy to help!

What is the cost to me?

There is no cost to you for services performed at the SBHC. We bill most insurances and we accept what we receive as payment in full. You are not responsible for co-pays, deductibles or balances.

What costs am I responsible for?

While most routine services performed at the SBHC are at no cost to you, there are some services offered that you would be financially responsible for, should they be recommended for your child.

These services will only be done with your permission:

  • Prescriptions
  • Lab work
  • X-rays
  • Specialty appointments
